All participants must be registered and are urged to register in advance. Non registered people will not have an access to the session halls and exhibition area.

Online registration – preferred
All participants are kindly recommended to register preferably via the Online Registration Form (not available as of 24^th August - only onsite registration is possible). Other registration possibilities are also available below. Please read the instructions on the conference website carefully.

Subscription as a member will only be approved with a valid membership number provided or new membership purchased (only until 1^st July 2009) at the same time.

Other registration possibilities
If you have no access to the Internet, please send the Registration Form for reduced registration fee to the Conference Department in Prague by 1^st July 2009.
A PDF version of the Registration or Accommodation Form can be downloaded on Conference website and are to be used for registration by fax or airmail only.
Each participant submitting the Registration Form by fax or airmail will receive a PRN (Participant Registration Number) by e-mail to include in details of payment.

Please do not send any payments (Bank transfers) without PRN included in details of payment. Such payments cannot be identified.

All participants and accompanying persons are kindly requested to register in advance by completing and submitting the Registration Form with the appropriate fees.
If your payment is not received before deadline dates 1^st July, 10^th August 2009 the late fee will be automatically charged.

After 1^st July 2009

After 10^th August 2009
You are kindly requested to make all payments by credit card exclusively or to register on-site. Payment by bank transfer is available only to this date.

After 23^rd August  2009
Pre-registrations will NOT be accepted any longer by the Conference Department BUT you will have the possibility to register onsite.

Registration fee includes:

Accompanying person fee includes:

• Access to exhibition Opening Ceremony
• Welcome reception at the Congress Center Hamburg
• Classical Sightseeing Tour of Hamburg , 7.9.2008, 13:00-16:00
• All Accompanying Persons are asked to re-confirm attendance on this tour by mail to Conference Department  This e-mail address is being protected from spam bots, you need JavaScript enabled to view it , then they will receive a voucher for this tour with conference badge
• Please note that Accompanying persons are not allowed to attend the scientific sessions and access to exhibition hall

Confirmation
Upon receipt of the correct registration fee (clear of bank charges), each participant will receive a confirmation of registration. It is obligatory to bring this confirmation to the registration desk at the Congress Center Hamburg as proof of your registration otherwise we cannot identify  your pre-registration if any problem occure. 
Confirmation will be sent upon receipt of payment. Please note that registration for the various tours and events will be confirmed upon payment on “a fi rst come, fi rst served” basis, and numbers may be restricted.

Payment
All fees for the Conference registration should be paid in EUR (€), free of all bank charges!

• by bank transfer (clear of any bank charges) clearly stating the participant‘s name and PRN (Participant’s Registration Number)

Bank transfer can be made only upon issued advance invoice, payments received without pre-registration details (name, PRN, invoice) cannot be identified.

• by credit card (Visa, Eurocard/Mastercard)

Please note:

• If your payment is not received before deadline dates 1^st July, 10^th August 2009 the late fee will be automatically apply for all unpaid registrations or new requests
• Allowance of 10%  for name changes and cancellations for free on site.

The place of jurisdiction is the location of the Association, Zürich, Switzerland.

Cancellation / Refund Policy
All cancellations must be notified in writing (by mail, fax or email) to the Conference Department according to the following conditions.
Refunds will be processed AFTER the Conference. Requests for refunds made after 5^th October 2009 will not be considered.

Registrants may transfer their registration to a colleague at any time. For any change of name a fee of € 50 will be charged.
All registrations received at the Conference Department CZECH‑IN, by 23^rd August 2009 will be included in the list of participants.

Registration opening hours
All documents including registration packs (conference bag, personal name badge, abstract book, final programme) will be handed out to registered participants during the following opening hours:

Saturday, 5^th September 2009  12:00 - 21:30
Sunday, 6^th September 2009  07:30 - 18:00
Monday, 7^th September 2009  08:00 - 18:00
Tuesday, 8^th September 2009  08:00 - 13:00

Responsibility
The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the Conference be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the nonappearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.
